Profile: (Graduate) Nurse/ Implementation Consultant
The nurse will assure the implementation, execution and of the Care Paths management of eCare Centre or Clinic for Diabetes Mellitus Center.
Job Summary:
We are enrolling Diabetes Mellitus Centre program for Indonesia. With a target of 50 centres for the next 1,5 years.
The goals of the Care Path Implementation Consultant (IC) are to gather information and requirements to provide recommendations for configurations, and to ensure proactive communication to the team and client’s staff. Capability to understand the Care Paths process. Design/ Propose a functional solution to meet those needs.
The IC drives projects to completion by anticipating the needs of the clients and ensuring the alignment of the internal and external teams, vendors, and other stakeholders, that are involved and focused on the implementation.
The IC will play a critical role in the success of the Implementation and thereafter the operations of the clinic. As a start of Willing to learn and support the expansions to multiple sites and include Consulting activities in Java and future locations in Indonesia. We are a young, world-class professional organization engaged with international stakeholders and partners, and system integrators. This position is an individual contributor role within our Professional Organization and reports directly to the Project Director or management.
Job activities:
• Conducting assessment’s and gathering information for the Care Paths
• Facilitate and document discussions and whiteboarding for applications functionalities/ requirements
• Implement pilots and help customers envision health process, in this case DM-Centre
• Provide remote support of patients based on results filled out in issue tracking and monitoring applications, e.g. Jira.
• Training and Coaching customer local staff
• Assure the implementation of patient administration and health reporting for the clinic
• Understand the application, including the patient personal portal and mobile App. Including patient-related content and tasks, enable to transfer knowledge to local staff
• Continue in search for new site for DM-Centre program
• Establish and maintain relevant networks with local stakeholders at the national, regional, or local level
• Develop a “go to market” strategy for the PT IDBH portfolio
• Manage the process from lead generation to conversion to sales revenue, including managing expectations;
• Plan, develop, and carry out activities with the aim of increasing brand awareness and enabling IDBH to develop a sustainable business with the company portfolio.
• Support activities and activities regarding prospects, pilots, and ongoing projects.
m) Continue in search for a new site for the DM-Centre program
• Monitoring and conducting activities to support the marketing. E.g., social media etc.
Job Required Skills & Qualifications: • Candidates would have a “nurse” or “midwife” education, graduates are welcome
• Have strong communication skills, pro-active and speaks Bahasa Indonesia and English, also in writings
• Use to work with computers, mobile app and cloud applications
• Understanding the basic Care Paths of a Clinic process in general and specifically for and experience with Diabetes-Hypertensions is a pre.
• Experience in patient’s managing process
• Experience in managing routine control assurance process
• Customers staff education and support (in close collaboration with a doctor’s and midwives and other caregivers)
• Assure Publish (bi-)weekly, monthly and quarterly patient administration – and healthcare reports
